This Agreement is entered into between Your Studio Name and Your Student Name for participation in classes, instruction, tutoring, coaching, training, or other related educational services.

The specific class details, payment schedule, tuition structure, cancellation policy, make-up policy, deposit terms, refund policy, and any other applicable conditions are defined by the selections, entries, and additional terms included in this Agreement.

This Agreement becomes effective on the date re provides acceptance or signature, whether electronically or otherwise.

1. Classes
The Instructor agrees to provide classes as arranged between the parties. Class format, location, duration, frequency, schedule, and subject may be specified in the selected options, registration details, invoices, or other written communication.

2. Payment
The Student and/or Parent/Guardian agrees to pay all applicable fees according to the payment method, payment schedule, tuition amount, and related billing conditions provided in this Agreement.

3. Policies
All policies including cancellations, rescheduling, missed lessons, and make-up lessons will be communicated and are considered part of this Agreement.

4. Liability
Neither party shall be held responsible for any injuries, accidents, or losses except where required by law.

5. Governing Law
This Agreement shall be governed by the laws of the state in which the services are provided.

6. Acceptance
By electronically accepting this Agreement, re acknowledges that they have read, understood, and agree to the terms of this Agreement.

*If the student is under 18, acceptance must be provided by a Parent or Legal Guardian.

7. Changes to Terms
Tuition rates, fees, schedules, and all related policies are subject to change at any time with reasonable prior notice. Continued participation in lessons after such notice constitutes acceptance of the updated terms.
