eForms.us and Lesson Forms were created in 2026 by Centreville Music Shop, a music store and music studio that has been serving Centreville, Virginia for 19 years.

During the pandemic, we began offering online lessons and needed a simple way to manage lesson agreements remotely. We created our own online form system to handle agreements without requiring in-person meetings.

The system proved to be extremely convenient and effective for both customers and internal management. Because of its simplicity and flexibility, we decided to develop it into a platform that others can use as well.

Unlike traditional form providers, eForms.us focuses on essential options. By selecting only what applies to your situation, the system generates a form tailored specifically to your needs.

In real-world use, different teachers may have different lesson rates, lesson durations, and payment structures. With eForms.us, a single form can adapt to all of these variations. Agreements can be sent by email and approved within minutes, even for customers who are located far away. Once approved, a completed PDF agreement is automatically delivered to both parties.

Lesson Forms is the first product launched through eForms.us, built from years of hands-on experience. We will continue developing practical and easy-to-use forms to support a wide range of businesses.

A common question is whether clicking a button or typing a name online is as legally binding as a handwritten signature. Under the Electronic Signatures in Global and National Commerce Act (ESIGN Act), the answer is a definitive yes.

The ESIGN Act was passed by the U.S. Congress in 2000 to facilitate the use of electronic records and signatures in interstate and foreign commerce. It established that a contract or signature “may not be denied legal effect, validity, or enforceability solely because it is in electronic form.”

According to the Act, an electronic signature is defined as an “electronic sound, symbol, or process, attached to or logically associated with a contract… and executed or adopted by a person with the intent to sign the record.”

This means that when a parent or student clicks the “I Agree” or “Accept” button in an email from LessonForms, they are engaging in a “process” that demonstrates clear intent to be bound by the terms. This digital action carries the same legal weight as a physical ink signature because our system captures that intent alongside a verifiable audit trail, including the signer’s IP address, date, and exact timestamp.

This interactive form allows you to create a customized agreement by selecting the options that apply to your services and entering your own specific details.

Once submitted, the agreement is prepared and delivered by email to the other party for review. The recipient can review and accept the agreement directly from the email, allowing for fast and convenient remote approvals.

In addition to remote email acceptance, the system can also be used in person. You may present the agreement on your own device, such as a phone or tablet, and have the student or parent review and accept it immediately during a lesson or meeting.

For those who prefer a more traditional approach, a professional PDF version of the agreement can be generated, downloaded, and printed for handwritten signatures. This flexibility allows you to use the system in a wide range of real-world situations.

After acceptance, both parties receive a completed PDF copy for their records.

All agreements you create are stored and can be accessed at any time through your My Agreements page. From there, you can review, download, and manage your existing agreements conveniently.

You can also download a CSV file containing all of your agreements and import it into tools such as Google Sheets or your CRM system, making it easy to organize and manage your students and records.

Agreements are delivered by email and can be accepted electronically. Once accepted, a PDF copy is generated and sent to both parties for recordkeeping.
