Lesson Forms by Centreville Music Inc. (“Lesson Forms,” “we,” “our,” or “us”) provides tools to create customized lesson agreements. By using this service, you agree to the following terms.

Lesson Forms provides an interactive platform for generating agreement documents. You agree to use the service only for lawful purposes and in accordance with these Terms.

Lesson Forms does not provide legal advice, legal services, or legal representation of any kind. Any agreement generated through this service is for informational and convenience purposes only and should not be considered a substitute for professional legal counsel. You are strongly encouraged to consult a qualified attorney to ensure that any agreement you create complies with applicable federal, state, and local laws before using it with any student, parent, or guardian.

You are solely responsible for reviewing, modifying, and ensuring that all agreement content, including policies and terms, is accurate, appropriate, and legally compliant for your specific situation. You acknowledge that any pre-filled content must be reviewed and replaced with your own policies where necessary. Lesson Forms makes no representations or warranties regarding the legal enforceability, validity, or sufficiency of any agreement created using our service in any specific jurisdiction or circumstance.

Lesson Forms is a neutral technology platform only. Any dispute, claim, or legal action arising between you (the instructor, studio, or school) and any student, parent, or guardian is solely between those parties. Lesson Forms is not a party to any such agreement or dispute and shall bear no responsibility or liability whatsoever for any claims, damages, losses, or legal outcomes arising from agreements created or executed through this platform, regardless of their nature or outcome. By using this service, you expressly release Lesson Forms from any and all claims related to disputes between you and your students or their guardians.

By using the service, you consent to receive communications related to your agreements, including emails and electronic confirmations. You acknowledge that electronic acceptance may be legally binding. However, Lesson Forms does not guarantee the delivery of any email notifications or agreement documents, and shall not be liable for any failed, delayed, or misdirected delivery due to technical issues, spam filters, or any other cause.

Certain features of the service may require payment. By submitting payment, you agree to the applicable fees. All payments are final unless otherwise stated.

Lesson Forms reserves the right to modify pricing, fees, and service plans at any time, with or without prior notice. Such changes may apply to new or existing users. Continued use of the service after any pricing or plan changes constitutes your acceptance of those changes.

Due to the nature of digital products and instant document generation, all purchases are final and non-refundable. Once an agreement has been generated or accessed, no refunds will be issued under any circumstances.

You agree not to initiate chargebacks or payment disputes with your payment provider for services that have been delivered. If you have any issues, you must contact us directly to resolve the matter. Unauthorized chargebacks may result in suspension or termination of your access to the service and may be subject to additional recovery actions.

Lesson Forms may update, modify, or discontinue features of the service at any time without notice. We do not guarantee uninterrupted or error-free operation of the service.

To the fullest extent permitted by applicable law, Lesson Forms shall not be liable for any indirect, incidental, special, consequential, or exemplary damages, including but not limited to, damages for loss of profits, goodwill, use, data, or other intangible losses arising from your use of the service.

In no event shall the total aggregate liability of Lesson Forms (including its officers, directors, and employees) for all claims, damages, and causes of action (whether in contract, tort, or otherwise) arising out of or related to your use of the service exceed the actual amount of the form generation fee paid by you for the specific transaction giving rise to the claim.

You agree to indemnify, defend, and hold harmless Lesson Forms, Centreville Music Inc., and their respective owners, officers, directors, employees, and affiliates from and against any and all claims, damages, losses, liabilities, costs, and expenses (including reasonable attorneys’ fees) arising out of or related to: (a) your use of the service; (b) any agreement you create, send, or execute through this platform; (c) any dispute between you and any student, parent, guardian, or third party; or (d) your violation of these Terms or any applicable law.

Lesson Forms may store agreement data and related information as part of its services. However, data storage, transmission, and access are not guaranteed to be error-free, secure, or uninterrupted. You acknowledge that technical issues, system failures, or unforeseen events may result in data loss, deletion, corruption, delay, or unauthorized access. To the fullest extent permitted by law, Lesson Forms shall not be responsible or liable for any such issues or for any resulting damages. You are solely responsible for maintaining your own copies and backups of any agreements or records generated through this platform.

The service may rely on third-party providers for email delivery, payment processing, or other functions. We are not responsible for the performance, availability, or policies of those third parties, and shall not be liable for any issues arising from their services.

We may update these Terms from time to time. Continued use of the service after changes are posted constitutes acceptance of the revised Terms.

These Terms shall be governed by and interpreted in accordance with the laws of the Commonwealth of Virginia, without regard to conflict of law principles. You agree that any disputes arising from the use of this service shall be subject to the exclusive jurisdiction of the courts located in Fairfax County, Virginia, and you hereby consent to personal jurisdiction in such courts.

These Terms, together with our Privacy Policy and any other policies referenced herein, constitute the entire agreement between you and Lesson Forms with respect to your use of the service, and supersede all prior or contemporaneous agreements, representations, or understandings.
